What Does Human-in-the-Loop Actually Mean?

In the early 20th Century, the British philosopher Gilbert Ryle coined the term “ghost in the machine”. Writing in The Concept of Mind, Ryle used the metaphor to push back against the mind-body dualism that maintains that mind and body exist as separate substances. For Ryle, this division was a mistake as cognition and physical action were inseparable, part of a single system rather than two interacting parts.
With the advent of AI, a similar metaphor now emerges when speaking of users of AI tools to increase productivity: the often-used “human-in-the-loop”. If humans and intelligent systems are now more fused than ever, are we building a seamless fusion or creating a convenient illusion of control?
Startups lean heavily on this concept to talk about their tools. While it promises both innovation and reassurance, the reality is often messier. Responsibility can easily become diffuse and accountability harder to trace.
As AI systems move deeper into sensitive domains—from education to warfare—the stakes are no longer abstract. What does human-in-the-loop actually mean and is this just a euphemism for when they disappear altogether?
1. Human-in-the-loop as a shield for responsibility
Used carelessly, the term human-in-the-loop can be an easy way to shift responsibility without truly engaging with it. As many are noticing, a human signature at the end of a process does not guarantee ethical integrity, especially if the underlying system is poorly designed or insufficiently understood.
Maysa Hawwash, founder and CEO of Scale X, has written on the slide away from responsibility and is blunt about the way the concept is often deployed. “It’s actually not unsimilar to other ways of burden shifting.” Hawwash told Startup Beat, using the example of how HR managers often use a sign-off policy to move the company away from liability. “If you have this policy and people read it and sign off on it then, as a company, technically you’re not liable, right?” she said.
What emerges is a pattern familiar across corporate systems where responsibility is displaced rather than eliminated. Hawwash sees this as the lazy way out which avoids critical thinking or understanding the areas where it can affect people or communities. “So you’re shifting the burden, and then it doesn’t matter if people understand the policy, it doesn’t matter if the policy makes sense.”
In this framing, “human-in-the-loop” risks becoming less about meaningful intervention and more about procedural cover. The danger here is not just semantic. When oversight is reduced to a sign-off, the human role becomes symbolic rather than substantive.
Hawwash referenced a recent military atrocity—the school in Minab, Iran—where humans approved a strike but the presence of a human decision-maker did not necessarily equate to ethical clarity or adequate deliberation. “When you’re in war or you’re conducting a complex surgery, you don’t have the luxury of time to use human-in-the-loop as a shield.”
2. Designing for responsibility, not just oversight
The alternative is not to abandon human-in-the-loop systems, but to take them seriously as design commitments. This means moving beyond symbolic oversight toward deliberate responsibility structures.
“There is this big race to get more AI right into the market. There’s not much thinking through from a design perspective, like what the downstream impact is on communities, on people or on end users,” Hawwash said.
Speed has become the dominant competitive variable. In that race, responsibility is often deferred rather than embedded. The result is a reactive model of ethics where fixing issues occurs after deployment instead of anticipating them during development.
Accessibility might accelerate adoption, but it also leads to more amplified consequences. Systems are no longer confined to technical users as they can shape decisions for people with varying levels of understanding and context. In such an environment, responsibility can’t be outsourced to the end user.
3. Human-in-the-loop as accuracy and accountability
Abhay Gupta, cofounder of Frizzle, offers a more operational perspective—one grounded in building a system where human oversight is both practical and necessary.
His company emerged from a specific problem: overworked teachers. “In the city you hear about bankers and consultants working 70 hours a week, but you don’t hear about teachers working that much. So out of curiosity, we interviewed hundreds of teachers and across the board grading was their biggest time sink.”
Automating grading might seem straightforward, but the complexity of handwritten math introduces real limitations for AI. “There’s the accuracy issue. AI isn’t perfect, so we built a human-in-the-loop system. If the AI isn’t confident—like with messy handwriting—it flags it for the teacher to review and approve or reject.”
Here, the human role isn’t just ornamental. The system explicitly identifies its own uncertainty and routes those cases to a human. “For us it’s about accuracy. There will always be edge cases—maybe 1–3%—where AI struggles, so a human needs to step in.”
This approach reframes human-in-the-loop as a mechanism for quality control. But Gupta pushes further: “At its core, AI isn’t 100% accurate—it can hallucinate or produce wrong outputs. Human-in-the-loop acts as the final quality check before results reach the end user. It’s also about responsibility. Someone has to be accountable for the output, and right now that still has to be a human.”
Importantly, the human role also preserves something less quantifiable: the relational aspect of teaching. “It’s also about preserving the human side of teaching. Teachers have different styles, so we let them customise how feedback is delivered”
Redefining Human-in-the-loop
The phrase “human in the loop” carries a reassuring simplicity. It suggests that no matter how advanced our systems become, a human remains in control and we aren’t simply “ghosts in the machine”. But as startups increasingly deploy AI in high-stakes environments, that reassurance demands scrutiny.
The deeper issue is design. If a system’s risks are poorly understood or intentionally minimized, inserting a human at the end does little to correct foundational flaws. Crucially, it also means defining the role of the human not as a fallback, but as an integral part of the system’s operation. A human in the loop shouldn’t merely approve the outcome. Startups should seek to empower their staff to shape them, challenge them, and, when necessary, override them with authority.
