Ellipsis Health announced on November 5, 2026 that it had acquired Kouper Health, adding native electronic health record (EHR) integrations and longitudinal patient maps to its Sage AI Care Manager. The companies aim to follow patients across care transitions instead of limiting support to isolated encounters.

Connecting Outreach to Appointments

Kouper develops AI agents for health-system and provider-group workflows. Its longitudinal patient maps consolidate information generated in different care settings, while its EHR integrations are intended to give Sage enough context to match patients with appropriate providers and book appointments directly in the record system.

Sage contacts people by phone and text for care-management enrollment, clinical assessments, medication adherence and post-discharge follow-up. The health-plan offering is designed to cover the full sequence from first member contact to a confirmed visit with a primary-care or specialty clinician, connecting patient engagement with the administrative work needed to complete that handoff.

Ellipsis founder and CEO Mainul Mondal said the deal is intended to help care teams reach more patients by adding infrastructure rather than increasing clinicians’ workloads. Kouper co-founder and CEO Salman Ali said the combined business is aimed at coordinating care over time and extending support beyond a single appointment or discharge.

Sage’s Technology and Reported Results

Ellipsis says Sage’s patented Empathy Engine was trained on more than 3 million labeled clinical conversations. The system analyzes vocal cues in real time to assess emotional signals and adapt its interactions with patients.

Across deployments, Ellipsis reports six-times faster care-management program enrollment, a 60% reduction in administrative tasks, task-completion rates up to 66% higher than human agents and up to sevenfold returns on investment.

Ellipsis describes its customer base as including seven of the top 20 US health plans, along with academic medical centers and integrated delivery networks.

Kouper’s Deployment History

Kouper was founded in 2022 and emerged from stealth on May 8, 2025 with $10 million in funding from General Catalyst, 25Madison and CVS Health Ventures. At launch, direct integrations with health systems supplied real-time data from emergency, inpatient and urgent-care settings, creating a patient history that could follow people after hospital-based events.

Its health-system customers were at different deployment stages. LifePoint had expanded its Kouper partnership across several hospitals; Henry Ford Health’s Mosaic Clinically Integrated Network was working with Kouper to support its Care Hub teams; and Ascension Saint Thomas was piloting the platform at its Nashville Midtown site.

Kouper reported at the time that deployment could take as little as four weeks and that its health-system partners had seen patient engagement increase by more than 50% and post-discharge and emergency-department follow-up appointments improve by more than 60%.

Ellipsis also had a previous financing relationship with one of Kouper’s backers. Its June 12, 2025 announcement disclosed a $45 million Series A led by Salesforce, Khosla Ventures and CVS Health Ventures alongside Sage’s formal unveiling. CVS Health Ventures was therefore a named investor in both companies before the acquisition.

Kouper’s team is joining Ellipsis. Both companies plan to continue serving existing customers under their respective brands while developing a combined product roadmap, and Ellipsis expects new health-system offerings in the months ahead.

The immediate task is to combine Sage’s patient engagement with Kouper’s care context and planned EHR booking while preserving customer continuity. The acquisition links two sides of the follow-up process; whether they become one reliable workflow while both brands continue serving customers is the central execution question.
